Assisted Living

We have 75 spacious private rooms with bath available for Assisted Living residents. Assisted Living provides that interim level of care between the Independent Living and the Health Care Center. The Assisted Living staff is able to manage those residents with mild dementia and/or forgetfulness. The Assisted Living resident should be capable of interacting appropriately with other residents and must have the cognitive ability to pull their call light for assistance should an emergency arise. Assisted Living offers a more structured environment than the Independent Living.

Private Pay Offers Three Levels of Care

Level Two

  • All basic services
  • Medication management by nurse
  • Set up for self shower twice a week
  • Set up for activities of daily living
  • Cueing and reminders
  • Simple redirecting

Level Three

  • All basic services
  • Medication management by nurse
  • Assistance with shower (15-20 min) and shaving twice a week
  • Minimal assistance with activities of daily living
  • Simple dressing
  • Stand-by assist with transfers if needed
  • Occasional assistance to and from dining room for meals
  • Incontinence care once per shift
  • Assistance with T.E.D. Hose AM and PM
  • Cueing and reminders
  • More intense redirection

Level Four

  • All basic services
  • Medication management
  • Showered by staff (30 min.) twice a week
  • Assistance with activities of daily living
  • Total dressing
  • One person assist with transfers
  • Assistance to and from dining room (wheelchair if needed)
  • Assistance into bed at night
  • Incontinence care two times per shift
  • Assistance with T.E.D. Hose AM and PM
  • Wanderguard available with doctor's order
  • Housekeeping twice a week
  • Colostomy and catheter monitoring
  • Complex redirection


  • Emergency call system
  • Private bathrooms, sit-down showers and safety equipment
  • Wall-to-wall carpet
  • Mini blinds and window treatments
  • Bring your own furniture or we'll provide a twin bed and a chair
  • Built-in closets and dressers
  • Individually controlled thermostat

The following basic services are included in the monthly room rate:

  • Licensed Nurse on duty 24 hours
  • 24 hour personal care assistance
  • Daily activities by trained activities staff
  • On-site Beauty and Barber Shop
  • Snacks and beverages available at all times
  • Weekly housekeeping and linen service
  • Laundry service
  • Full-time staff Pastor
  • Nail care
  • Monthly vital signs monitored by a nurse
  • 3 meals served daily
  • Hall checks during the night
  • Medications ordered from house pharmacy if requested. If the family chooses to get medicine from an outside pharmacy it will be the responsibility of the family to keep Stow-Glen supplied with all medication per doctor's order.

Prior to admission from the outside community into Assisted Living Level 4, the resident must first be admitted into the Health Care Center and evaluated to ensure they are appropriate for an assisted living setting.

Accepting Both: Private Pay & Medicaid Waiver Approved Residents

The Assisted Living Waiver program bridges the gap between Medicaid in-home care and nursing facility care for Medicaid approved individuals.

To be eligible for Assisted Living Medicaid Waiver you must be:

  • Approved for Medicaid
  • Able to pay a monthly room and board payment
  • In need of hands-on assistance with at least two of the following:
    1. Mobility / Transferring
    2. Eating / Preparing meals
    3. Bathing
    4. Grooming
    5. Toileting / Using the restroom
    6. Taking medication
    7. Dressing

Once eligible you will work with your Care Manager to create a plan tailored to meet your specific needs.
